The phone reads 25% but the cell voltage has already dropped past the safe threshold under load. One full discharge-charge cycle corrects the fuel gauge mapping and eliminates most post-swap shutdowns.

USB-PD fast charge not activating on the first cycle after replacement

After a cell swap, the charge IC may withhold USB-PD negotiation on the first cycle as a precaution while the BMS initialises against the new cell. You'll see slower-than-normal charging through the first full cycle — this is normal behaviour, not a fault. Once the BMS completes its initial calibration pass, fast charge resumes automatically on the second cycle. If it hasn't resumed by the third cycle, check that the Lightning cable and adapter are USB-PD rated.
