The battery gets slightly warm during the first charge after swapping — is that a fault?

Warmth during the first charge is normal for a fresh Li-Polymer cell going through its initial conditioning cycle. The charge IC may apply a trickle or pre-charge phase to a cell coming in from storage voltage, which generates mild heat. If the pack becomes hot to the touch or the device smells unusual, disconnect immediately. Gentle warmth that fades once the pack reaches full charge is expected behaviour and clears after the first cycle completes.

The capacity indicator still shows incorrect readings after the swap — how do I fix it?

The fuel gauge IC calibrates against the cell it last measured, so a new pack will produce inaccurate readings until it learns the new capacity curve. Run one complete charge from flat to full without interrupting the cycle. That single uninterrupted cycle forces the gauge to recalculate its endpoint voltages against the replacement cell, and the percentage display should track accurately from the second cycle onward.
