The battery I removed has visibly swollen — is that a cell fault or a fitting fault?

Swelling in a Li-ion cell used in emergency lighting is almost always caused by prolonged trickle overcharge, not a defective cell. Ceag fittings that have run for several years can develop charge controller drift, where the float voltage creeps above the safe ceiling and slowly overcharges the cell through repeated shallow cycles.
