The Rescue Sam won't turn on after the ELRS-002 sat unused for several months — what's happening?

Li-MnO2 cells self-discharge in storage, and the Rescue Sam's BMS refuses to boot if cell voltage has dropped below its recovery threshold — approximately 10.5V. The fix is to charge the battery fully before attempting to power the device on. Once voltage is back above the threshold, the unit will boot and complete its self-test normally.

The Rescue Sam passed its self-test after the battery swap, but the charge indicator never reached 100% on the first charge — should I be concerned?

No. The charge IC in the Rescue Sam applies a conservative current limit on an unrecognised new cell and will stop short of 100% on the first pass.
