This happens when the cell's resting voltage at delivery is below the alert threshold the hub monitors — common with Ni-MH after extended shelf storage. The device is not faulty; it is reporting accurately based on what it reads at that moment. Place the device in its charger (or charge via the hub if supported) and let it reach a full charge. The alert clears once the cell voltage rises above approximately 6.8V under light load.

My Bosch D14 showed up as offline in the app right after I put the new battery in — did I fit it wrong?

The battery is almost certainly fine. Bosch smart home devices run a mesh network re-join sequence after any power interruption, and that process takes up to 60 seconds. Wait a full minute after fitting the cell before checking the app. If the device is still offline after 60 seconds, trigger a manual re-scan from the hub's device list.

The new battery is draining noticeably faster than the old one did in its first few weeks — is something wrong with the cell?

Likely not the cell. A freshly installed Ni-MH battery that arrived below operating voltage causes the D14's power management circuit to increase its polling rate, which drives higher drain for the first 24–48 hours. Run one full charge cycle and monitor drain over the following two days. If it stays high after that, check the hub's polling interval setting — frequent sensor queries on a busy mesh network will shorten any cell's life.

The hub completely lost my K12 sensor after the battery swap — it won't show up at all, not just offline

This is a different issue from a standard re-join delay. When a Bosch hub loses a device entry entirely, the hub dropped the pairing record during the power gap rather than just the connection. Open the Bosch Smart Home app, go to Devices, and use Add Device to re-pair the K12 from scratch. Have the device within 2 metres of the hub during pairing to avoid a signal-strength failure mid-handshake.
