The fault LED on my fitting is still on after I installed the new RT-146 pack and confirmed the connections are solid — how do I clear it?

Some emergency lighting controllers latch a fault state in firmware and do not auto-reset when a new cell is connected. Disconnect mains power, wait 30 seconds, then reconnect — this forces the controller to re-poll the cell voltage and re-enter its charge cycle from the start. If the fault LED persists after that reset, confirm the pack resting voltage reads at least 4.2V with a multimeter before suspecting the controller itself.

The old battery had swollen and was pressed tight in the fitting — is that a cell failure or a fitting problem?

Swelling in Ni-MH packs is caused by sustained overcharge, which generates internal gas pressure over time. In emergency lighting fittings, this usually happens when the charge controller's trickle rate is set too high for the cell capacity, or the fitting is mounted in a high-ambient-temperature location with no airflow. Replace the pack, then check that the fitting is not enclosed in a sealed housing that traps heat — Ni-MH cells tolerate continuous float charge much better below 35°C. If swelling recurs within a year, the charge controller itself needs inspection.
