The device reads this as a weak or discharged pack, not a fault. Charge the pack fully before first use and the voltage will settle at the rated 4.8V across all four cells.
Pack not reaching full charge on the first cycle
Many charger ICs reduce current on the first charge of a new or long-stored Ni-MH pack — this is the charge controller being cautious with cells it hasn't profiled yet. The pack is not faulty. After the first complete charge cycle, the charge IC adjusts its current delivery and the pack accepts full capacity from cycle two onward. If capacity still reads low after three full cycles, check the charger output is delivering the correct termination voltage of at least 5.5V to the pack terminals.

The device powered on, detected voltage below its low-battery threshold, and cut off to protect itself — it's not a fault with the pack. Charge the pack fully before re-installing it. Once the cells reach 4.8V nominal, the device will stay on through a normal cycle.
My charger is running for much longer than usual on this pack's first charge — is something wrong?
No. Many charge ICs throttle current on the first charge of a new Ni-MH pack because the cells have no charge history for the controller to reference. The controller is running a slow conditioning pass, not stalling. Let it complete without interrupting. From the second charge onward, charge time will normalise as the IC builds a profile for the cells.
The pack feels noticeably warm during the first charge — should I pull it off the charger?
Mild warmth during the first conditioning charge is normal for Ni-MH chemistry. The cells generate heat as they accept charge from a depleted storage state, and Ni-MH packs use heat detection (delta-temperature termination) as part of the charge-end signal. If the pack becomes hot to the touch — uncomfortable to hold — remove it and let it cool before retrying. A warm pack that finishes the cycle cleanly and cools within 15 minutes of charge completion is behaving correctly.
